Wellbeing Program Application - For Individuals


We’re really excited that you’ve found us and are keen to know more about our wellbeing program for individuals. It’s a great introduction to the farm if you’re experiencing mental health concerns. Our programs for individuals run for six weeks, typically on Friday mornings from 9am - 12pm and are currently offered FREE of charge.

Our next program commences 18 October - 22 November 2019.

On entry into the program we will work with you to complete a personal wellbeing plan to ensure that you’re getting the most out of what we can offer. And we’ll support you throughout the course to track your progress towards the goals you set. 

On completion of the program you are welcome to join our weekly volunteer sessions and continue growing and harvesting produce. You may also be able to participate in future wellbeing programs as a participant or mentor.

Whether you have little or lots of experience, if you’re interested in gardening, our wellbeing program aimed at promoting positive mental health is for you!


Applicants must meet the following criteria to participate in our wellbeing program:

  1. Be aged 18 years or over — participants can enter the program on or after their 18th birthday. There is no upper age limit. 

  2. Be self-referred or referred to the program through a social or medical services provider — North Fremantle Social Farm accepts referrals from GPs, specialists, and community and clinical social and health services.

  3. Be able to participate in all aspects of the program — participants must be physically and mentally capable of participating in all activities, and be willing and capable of receiving and acting on instructions.

  4. Maintain an interest in gardening and demonstrate a commitment to undertake the program — wellbeing programs are voluntary & participants cannot be forced to attend or participate. Applicants must demonstrate an interest in gardening, although no prior experience is necessary, and a commitment to undertake the program for its duration.

  5. Commitment to adhering to North Fremantle Social Farm’s Code of Conduct — participants are required to adhere to a Code of Conduct, with any breach resulting in possible termination from the program.

  • North Fremantle Social Farm is a drug, alcohol and smoking free zone

  • No domestic animals are permitted on site. Though, support dogs may accompany owners with prior arrangement


You may find the form pretty long and some questions might be a bit confronting. Please be assured that your personal information will be treated confidentially. It will only be used for the purposes of assessing program readiness, assist us to design the program according to your needs if approved, and help us to manage any potential risks.
